How to get a car accident report in Arizona

The short answer
To get a copy of an Arizona Department of Public Safety crash report, go to the DPS Public Services Portal and select the request a report or records option. A police officer who investigates a crash must complete a written report when someone is injured or killed, property damage is more than $2,000, or a citation is issued. Smaller crashes still get a short report with names and a description.
Which agency has your report
The report comes from the agency whose officer came to the crash. On interstates and state highways that is usually the state police or highway patrol. On city streets it is the city police, and outside city limits it is often the county sheriff. Ask the officer for the report number before you leave the scene.

Whether you must file your own report
Arizona does not require the driver to separately file a written crash report with the state. The investigating officer must complete a written report within 24 hours of finishing the investigation and immediately send it to the Department of Transportation, if the crash caused injury, death, property damage over $2,000, or a citation.
“A law enforcement officer or public employee who, in the regular course of duty, investigates a motor vehicle accident that results in... damage to the property of any person in excess of $2,000... within twenty-four hours after completing the investigation, shall forward the report to the agency employing the officer or public employee, which shall immediately forward a copy of the report to the department of transportation.”
